Summary of Roles and Responsibilities:
* Support individuals with their personal, social, emotional, and practical needs.
* Develop trusting relationships and provide person-centred care.
* Promote independence, dignity, choice, and inclusion.
* Create and implement individual care plans in partnership with service users and multidisciplinary teams.
* Safeguard vulnerable people by recognising and reporting concerns about abuse or neglect.
* Assist with daily living activities such as personal care, meal preparation, budgeting, education, and community access.
* Encourage positive behaviour and provide emotional support during challenging situations.
* Maintain accurate records, write reports, and document observations.
* Communicate effectively with families, healthcare professionals, and other agencies.
* Follow relevant legislation, organisational policies, and professional codes of practice.
* Participate in team meetings, supervision, and ongoing professional development.
* Advocate for service users' rights, ensuring their voices are heard and respected.
* Support crisis intervention and risk management when required.
* Promote health, safety, equality, diversity, and confidentiality in all aspects of care.
